Player Requirements:
A.) Liability Waiver & Agreement
Parties failing to complete and submit this agreement are NOT allowed to participate in any capacity of Dallas Diesel SPORTS.
Part 1.
In consideration of the acceptance for initial workouts and participation with or as a member of the Dallas Diesel SPORTS Club, I do hereby, for ourselves, heirs, executors, and administrators, waive, release, and forever discharge any and all rights and claims for damage which we may have or which may hereafter accrue to us against the Dallas Diesel SPORTS Club and our facilities, owners, and/or any of its teams, the coaches, sponsors, administrators of the playing field, or their respective officers, agents, or representatives, successors, and/or assigns for any and all damages which may be sustained and suffered by us in connection with our said association with or entry, and/or arising out of our traveling to, participating in, and returning from the Dallas Diesel SPORTS Club event and/or facilities, games, practices, or exhibitions conducted.
Part 2.
I agree, by submitting this electronic form, to follow the rules and bylaws of the Dallas Diesel SPORTS Club and facilities. I also authorize the Dallas Diesel SPORTS Club to distribute the my information to sports agents, talent scouts, and professional teams for possible future contact regarding tryouts, camps, team visits, high levels of play, and/or marketing opportunity.
- Player must be between 5 - 13 for Youth Sports, 14-17 Jr Sports, 18 & up for adult sports.
- All parents must sign/complete the Dallas Diesel SPORTS Club Team liability waiver below.
- Player must be In good health and physical condition to compete at a vigorous levels of play.
- Players are absolutely not allowed to play while pregnant.
- Drug–Free, no illegal drug use. Not under the influence of Alcohol or ANY drugs (including medication)

Membership Fee: $10/ Month + pay per event Normal Practice Times: Wednesdays (Youth) 6:30 - 7:30p Wednesdays (Adult Men & HS Boys) 7:30p - 9:00p Thursdays (HS Girls) 6:00p - 7:30p Thursdays (Adult Women) 7:30p - 9:00p Home Gym Location: First Baptist Church of Richardson Activity Center 1001 N. Note: * * Affiliated team memberships vary * * * Dallas Diesel Apparel based on availability. * WBCBL play is only for selected Adult female players. * Monthly membership fees are due on the 1st of each month. * Monthly membership fee required prior to participating in any events. * Membership based on compliance of Dallas Diesel Member Agreement. * Local league play consists of 2 to 3 area competitive leagues * Road trip expenses (hotel, meals, etc..) are in addition to club membership (not included) but may be covered through club fundraisers and club activities. * * * * * All Membership and Event fees are non-refundable. * * * * * |
