In consideration of the acceptance to participate with or as a member of our team, I do hereby for ourselves, our heirs, executors, and administrators, waive, release, and forever discharge any and all rights and claims for damage which we may have or which may hereafter accrue to us against Dallas Diesel & Conway Bearcats Sports Club and/or any of its teams, the coaches, the sponsors, the administrators of the playing field and facilities, or their respective officers, agents, or representatives, successors, and/or assigns for any and all damages which may be sustained and suffered by us in connection with our said association with or entry, and/or arising out of our traveling to, participating in, and returning from said organization events, games, practices or exhibitions conducted during the season.
 
By submitting this waiver, the child and I agree to follow the rules and bylaws of our organization. Anyone that does not sign this agreement is not eligible to participate.
 
By submitting below you are authorizing the use of all photographs, videos and likeness of you/your child to be used and reproduced for Marketing and Promotional purposes for the Dallas Diesel & Conway Bearcats Sports Club.
